Ileocolic intussusception and hydrostatic reduction in a 2-year-old girl.

چکیده
DESCRIPTION A 2-year-old previously well girl presented at the hospital with intermittent acute painful crying (colicky pain) that recurred at frequent intervals for some hours. Between these episodes, she was painless but refused to eat or drink. During palpation, a mass was suspected in the right hemiabdomen. The rectal examination was unremarkable, and no bloody diarrhoea was present. Abdominal sonography with a linear transducer revealed an ileocolic intussusception (figure 1). For hydrostatic reduction, warmed saline was instilled in the rectum. The intussuscepted ileum was smoothly pushed back under ultrasound guidance. Figure 2 shows the situation after hydrostatic reduction, with saline flowing from the cecum through the thickened ileocecal valve into the terminal ileum. Intussusception describes the telescoping of a bowel segment into an adjacent segment. Ileocolic intussusception is the most common cause of intestinal obstruction in children between 3 months and 6 years of age, with an incidence of 0.3–4 per 1000 individuals. 3 In this age group, most cases are idiopathic without a pathological lead point, apart from a possible association with hypertrophied lymphoid tissue in the terminal ileum. Untreated intussusception is often fatal since most cases do not resolve spontaneously. With a timely diagnosis, many patients can be treated successfully without surgery. This may be performed by air enema or contrast fluid enema under x-ray fluoroscopic control. 3 A radiation-free alternative is hydrostatic reduction under sonography-guidance. If such pneumatic or hydrostatic reduction is impossible or contraindicated, then surgery is required.

Chronic Intussusception Caused by Diffuse Large B-Cell Lymphoma in a 6-Year-Old Girl Presenting with Abdominal Pain and Constipation for 2 Months
The classical triad of abdominal pain, vomiting, and bloody stool is absent in chronic intussusception for more than 2 weeks. Here, we report a 6-year-old female with recurrent abdominal pain for 2 months. Ultrasonography of the abdomen revealed an ileocolic-type intussusception. The lesion accompanying the tight fibrous adhesion was treated by resection and ileocolic anastomosis. It was diagno...
متن کاملChronic ileocolic intussusception due to transmural infiltration of diffuse large B cell lymphoma in a 14-year-old boy: a case report
Chronic intussusception, defined as intussusception continuing over 14 days, is rare in children. We herein report a case of chronic ileocolic intussusception caused by the transmural infiltration of diffuse large B cell lymphoma in a 14-year-old boy. The patient had been suffering from anorexia and intermittent abdominal pain for 5 weeks, during which his body weight decreased by around 7 kg. ...
متن کاملIleocolic intussusception due to Burkitt lymphoma: a case report
Burkitt lymphoma has many forms of clinical presentations and, in children, it is usually discovered due to the presence of an abdominal mass. This rapidly growing tumor is highly malignant, aggressive, and may cause either indirect symptoms, due to pressure phenomena or direct involvement of the bowel lumen, leading to either intestinal obstruction or intussusception. We describe the case of a...
متن کاملUltrasound-Guided Hydrostatic Reduction of Childhood Intussusceptions Using Water Enema
Conventional hydrostatic barium reduction or pneumatic reduction of intussusception is associated with considerable ionizing radiation. The aim of this study was to evaluate ultrasound-guided hydrostatic reduction for childhood intussusception using water enema. A total of 76 children who diagnosed as having intussusception were referred to Doctor Sheikh Children Hospital in Mashhad (northeast ...
متن کاملRecurrent Intussusception in Infants
Background The majority of intussusceptions, between 2 months and 2 years, are idiopathic. Indication of surgery in infants with recurrences is still matter for debate. The aim of our study was to review our experience to better define the management of recurrent intussusceptions. Material and methods A retrospective study was conducted involving all patients treated for intussusception at the ...
